It treats focal seizures in adults and children above four years of age.  Lacosam 100 Tablet inactivates the sodium channels across the neuron membranes. This inactivation decreases neuron signals, thereby decreasing the electrical disturbance or abnormal activity in the brain.

✔️ Indications of Lacomax 100

Lacosamide is used in the treatment of epilepsy/seizures.

✔️ Pharmacology

Lacosamide works primarily by inhibiting sodium channels, which may contribute to seizure control by selectively targeting depolarized neurons, particularly those involved in pain and hyperexcitability. Additionally, Lacosamide binds to collapsin response mediator protein-2 (CRMP-2), a protein involved in neuronal development, although its exact role in seizure control remains unclear.

✔️ Dosage & Administration of Lacomax 100

• Adults (17 years and older):
  • Monotherapy: Start with 100 mg twice daily, with a maximum dose of 200 mg twice daily.
  • Adjunctive therapy: Start with 50 mg twice daily.
• Pediatric Patients (4 to less than 17 years):
  • Dosage is based on body weight and should be increased gradually based on clinical response and tolerability, no more frequently than once per week.
• Renal impairment: Dose adjustment is recommended for severe impairment.
• Hepatic impairment: Dose adjustment is required for mild to moderate impairment; Lacosamide is not recommended for patients with severe hepatic impairment.

✔️ Interaction

• CYP3A4 and CYP2C9 Inhibitors: Patients with renal or hepatic impairment who are taking strong inhibitors of these enzymes may require dose adjustments due to increased exposure to Lacosamide.
• Concomitant Medications that Prolong PR Interval: Use Lacosamide cautiously in patients taking beta-blockers or calcium channel blockers, as there is a risk of AV block or bradycardia. ECG monitoring is recommended before and during treatment.

Pregnancy: Based on animal data, Lacosamide may cause fetal harm.

✔️ Precautions & Warnings

• Monitor for suicidal ideation and behavior.
• Dizziness and ataxia may occur.
• Cardiac Rhythm Abnormalities: ECG monitoring is recommended in patients with known cardiac conditions or those on drugs that prolong the PR interval. Lacosamide may cause syncope and should be used cautiously.
• Withdrawal: Gradual withdrawal is recommended to minimize increased seizure frequency.
• DRESS/Multiorgan Hypersensitivity: Discontinue use if no alternate cause is found.
